The Formidable form feature on our blogs is good for when you want qualitative feedback on very focused questions. We created one form for patrons wishing to register their child/ren in the library’s Birthday Book Club.
Scrolling all the way down to the bottom of the editor page for Formidable allows you to set Form Notification options. I customized my form to send me an email when someone fills out the form. That email summarizes their answers for me. Once a user clicks on the submit button, they get a message thanking them and asking them to pass by the library to pay the Birthday Book Club fee.

In case you're wondering, the results page looks like this. We can use this at the end of the year to know how successful our program has been, but the emails are actually more useful for us in the selecting and purchasing of the books.
